IMPORTANT: MONTHLY MEMBERSHIP CONTRACT INFORMATION
Membership is the most economical way to practice at FORM yoga. We can offer memberships as an act of reciprocity. In exchange for a commitment of your studentship we offer you a lower price to practice. Please read and acknowledge our membership terms and conditions before purchasing.
- Our monthly memberships auto-renew on your credit card on the same day each month. Membership will auto-renew until we receive a cancelation request (see below).
- Each membership has a minimum 3-month minimum commitment, you may cancel anytime after the 3 months by submitting a Membership Change Request Form.
- All changes and cancelations require a 30 day advance notice.
- Your classes or membership perks may not be shared.
- There are no refunds or reimbursements or rollovers for unused classes or time.
- Memberships may be frozen once per year and will incur a $25 administration fee for each month the membership is frozen.
- You may change your tier membership anytime after the initial 3-month commitment.
- If your membership includes BFF passes they may not be used by the membership subscriber, only guests.
- BFF passes renew on the 1st of each month and expire at the end of each month.
- To use a BFF pass follow the instructions here.
- Unused BFF passes do not rollover to the next month.
- If you register your BFF for class in advance the pass is considered used.
- Studio policies, scheduled classes, class styles, and teachers may change throughout the course of your membership as normal business operation.There will be no refunds or reimbursements due to scheduling changes.
